Athletic Training

Dear Student-Athletes and Parents/Guardians:

I’d like to take this opportunity to welcome you to SMCC Athletics! Athletic Training coverage is provided for all practices and competitions for all 7 sports offered here at SMCC (men’s & women’s soccer, baseball, softball, men’s & women’s basketball, and golf). The athletic training facility is located within the HUB (Hutchinson Union Building) and is open, by appointment, before, during, and after all sporting events.

In accordance with USCAA and Southern Maine Community College Department of Athletics policies, all student-athletes are required to provide medical information and must be cleared by the Athletic Training Staff prior to their participation in pre-season and/or regular-season activities. Please note that your son or daughter will not be able to participate in any activity (practice, conditioning, games, etc.) until we have ALL of their forms on file. Therefore, please return these forms to the Athletic Training Department prior to your son or daughter’s arrival on campus. Note: this will be earlier than the start of classes for fall sports with preseasons (men’s and women’s soccer). Any information provided is considered confidential and will be kept solely by the Athletic Training Staff. The following information is required from student-athletes each year:

III. Athletic Training Authorization for Release of Information
IV. Acknowledgements, Consents and Releases Forms
V. Under 18 Consent Form (if applicable)
VI. Baseline Neurocognitive Concussion Testing
VII. Concussion Information (To be completed by all first-year and transfer student-athletes AFTER Aug. 1)
